Bad Boss

My boss makes bad decisions. She takes credit for the good work others do and blames other for her bad work. She’ll begin a project, decide it’s unworthy and rather than discard it, she’ll dump it on someone else and blame everyone else except herself. Should we do an intervention? Or should we talk to her boss? I’m at a loss.
